Chronically Homeless Outreach Partnership (CHOP)

To further promote a cooperative approach to livability issues, the Livability Team formed the Chronically Homeless Outreach Partnership (CHOP) in 2019 to bring together community stakeholders. The Medford Police Department along with multiple community partners developed a more focused and individualized approach to address the needs for the high-risk population within the City of Medford.

 

This partnership includes social service agencies, the courts, and non-profit organizations. The group meets monthly to discuss and create pathways for personalized solutions.
