Submit a Claim Against the City

To submit a claim of negligence against the City of Medford please take the following steps.

Step 1.Request a Form

Email the City Attorney's Office at legal@cityofmedford.org to obtain a blank claim form.

Step 2.Gather Your Information

The claim form must include the following information:

  • The claimant's name, address, and telephone number 
  • The location where the incident occurred;
  • If relevant, the Medford Police Department case number;
  • A description of what happened, and why you think the City if responsible;
  • Receipts, repair estimates, photos, and any other supporting documentation: and 
  • The amount you are requesting the City to pay.

Step 3.Submit Form

Once you have completed the claim form and gathered all supporting documentation, please return documentation to the City Attorney's office through the following ways:
