Child Care Home Based

Do you operate a child care center from your home here in Medford? Please follow the instructions below when you apply for your business license. *If you have a child care center located at a commercial location, you will need to apply for a Commercial business license. Information can be found on the main Business License page*

Here is what you need!

Step 1.Application

You will need to submit the following items for your home based child care business license:
1. Medford business license application (filled out entirely - if there is a question that does not pertain to you, please list "n/a" as your answer)
2. Home Based License Agreement
3. Medford Fire Emergency Information check list
4. Proof your business is registered or certified as a family child care facility. (Proof of State licensing)
5. Floor Plan with egress plan

Step 2.How to submit your application

Four ways to submit your business license application packet:
1. Email: businesslicenses@cityofmedford.org
2. In person: 200 S. Ivy Street, Lausmann Annex, Second Floor
3. By mail: City of Medford Attn: B/L, 200 S. Ivy Street, Medford, OR 97501

Step 3.Approval and Payment

Review process of your child care application takes approximately 2 - 7 business days
Once we have your information entered in, we will contact you by email with a link for payment
Annual fee follows the General Business License fee schedule
Payments can be made online, in person or by mail
We accept Visa, MasterCard, check or cash
