Foster Care or Residential Treatment Home Based

Foster Care or Residential Treatment businesses in a home (residential location), are required to have a business license in Medford.  You will find all the information on how to apply in this section.

Here is what you need!

Step 1.Application

  • You will need to submit the following items for your Foster Care or Residential Treatment business license:
    1. Medford business license application (filled out entirely - if there is a question that does not pertain to you, please list "n/a" as your answer)
    2. Medford Fire Department Emergency Information Form
    3. If your location has five (5) bedrooms or more, you are required to submit a copy of a current floor plan (*additional $50 fee will be due at time of application submission*)

Step 2.How to submit your application

  • Email:  businesslicenses@cityofmedford.org
  • In person:  200 S. Ivy Street, Lausmann Annex, Second Floor
  • By mail:  City of Medford Attn: B/L, 200 S. Ivy Street, Medford, OR 97501

Step 3.Approval and Payment

  • Review process of your application takes approximately 3-10 business days
  • Once we have your information entered in, we will contact you by email with a link for payment
  • Payments can be made online, in person or by mail
  • We accept Visa, MasterCard, check or cash
  • Annual fees are based on the General Business License Fee Schedule
