Foster Care or Residential Treatment Home Based

Foster Care or Residential Treatment businesses in a home (residential location), are required to have a business license in Medford. You will find all the information on how to apply in this section.

Here is what you need!

Step 1.Application

You will need to submit the following items for your Foster Care or Residential Treatment business license:
1. Medford business license application (filled out entirely - if there is a question that does not pertain to you, please list "n/a" as your answer)
2. Medford Fire Department Emergency Information Form
3. If your location has five (5) bedrooms or more, you are required to submit a copy of a current floor plan (*additional $50 fee will be due at time of application submission*)

Step 3.Approval and Payment

Review process of your application takes approximately 3-10 business days
Once we have your information entered in, we will contact you by email with a link for payment
Payments can be made online, in person or by mail
We accept Visa, MasterCard, check or cash
Annual fees are based on the General Business License Fee Schedule
